IMPORTANCE OF EARLY DETECTION OF KERATOCONUS: ABOUT 8 CASES OF CORNEAL HYDROPS / BOUHAJBANE YOUSSEF

صلبة؛ زراعة القرنية.Résumé : INTRODUCTION: Keratoconus is a corneal disorder defined by progressive ectasia and thinning. Advances in the diagnosis and treatment of keratoconus have an important impact in preventing its complications, including acute corneal hydrops as a major challenge, in which management is a crucial and difficult part.
RESULTS: Our study sample included 8 cases of corneal hydrops. 6 of our patients were seen at the acute stage of corneal hydrops. The mode of presentation in all our patients was corneal hydrops. The disorder was unilateral in 6 patients, in which 5 had signs of advanced keratoconus in the other eye. 2 patients presented at the residual phase (neovascularization/fibrosis) had bilateral involvement, suggesting a different timing of disorder. 5 patients had Down syndrome, and 3 patients had signs of VKC (vernal keratoconjunctivitis). All patients were treated medically at the acute phase. 4 patients underwent surgical treatment consisting of pre-Descemet's sutures and pneumo-descemetopexy. The evolution was marked by partial regression of hydrops by the second day and complete regression at 20 days under combined medical treatment. All our patients were proposed for penetrating keratoplasty. One patient underwent treatment for corneal neovascularization before keratoplasty.

The causes include infectious and noninfectious etiologies including immune-induced corneal ulcers. Both etiologies can lead to severe complications such as corneal melting, necrosis and perforation. When conventional therapies fail, emergency measures like penetrating keratoplasty become necessary to preserve ocular integrity and eradicate the pathogen in infectious cases. Our study investigates the outcomes of emergency penetrating keratoplasty for advanced keratitis cases, focusing on anatomical restoration, therapeutic efficacy, and functional outcomes. We found that while anatomical and therapeutic goals were partially achieved, visual outcomes remained poor, with a high incidence of postoperative complications. Our findings underscore the challenges associated with delayed surgical intervention and emphasize the importance of early consideration of surgical options in refractory cases to improve patient outcomes.
